Congregational Care Network Team
Second Presbyterian Church is proud to partner with IU Health as part of the Congregational Care Network (CCN). Together with other Christian, Jewish, and Muslim congregations, we are addressing loneliness and social isolation in our community.
What is CCN?
CCN provides support to IU Health patients, called companions, who self-identify as lonely or in need of additional help after being discharged from the hospital. Volunteers, known as connectors, are trained to:
- Provide compassionate, listening support.
- Help companions ease back into their at-home routines.
- Identify sources of support within companions’ existing networks.
- Connect companions to new resources and community opportunities.
Volunteer as a Connector
Prospective CCN volunteers receive comprehensive training, including:
- Listening skills.
- HIPAA compliance and confidentiality.
- Understanding community resources.
After training, connectors are matched with companions for ongoing, meaningful conversations that make a real difference.
